Software Zone Failure and Safe Practices

Has there ever been a documented case of a laser scanning outside of set zones as a result of a Beyond software failure?

I am dealing with a laser inspector in the UK forcing me to hard mask every laser. The problem is when I pull the shutter up so that the laser is completely blocked in the top seats, the shutter is blocking half the beam. So now the shutters are cutting off half my beam intensity on all my lasers to meet this requirement. Their argument is if there was a software failure, the beam would be blocked. I have never heard of a software failure like this, and I cannot find anyone who has.

This requirement also makes it physically impossible to terminate on balconies at all, because there’s no way to hard mask that precisely with shutters without cutting off most of the beam coming out of the aperture.

I am all for safe practices and following common sense rules and regulations, but this seems a bit too far.

Has anyone else had to deal with this?
